Supplemental Application Forms
Choose forms for download and complete for your audition application. These documents must be mailed to the Admissions Office at Mississippi School of the Arts after the completion of the online application. A PDF reader is required to view documents.
Recommendation Forms
Starting with the class of 2027, only a principal or counselor recommendation letter is required when applying for audition.
